A letter of invitation from the medical institution is mandatory.
Evidence of the covered medical treatment is also required.
Original copy of birth certificate should be tendered if applicant is a minor.
Letter from local doctor stating reason ailment cannot be treated in South Africa.
You are expected to return to South Africa after the end of the treatment.
Holders of this visa are not allowed to go sightseeing during their stay in the country.
The visa fee of this type of visa is $160.
The visa is processed for about 15 days and is valid for 3 months.

A valid passport with at least 3 months validity before return date are required. The passport should have at least two blank pages.
Two recent passport photographs with a light background that follows the passport format of a Denmark visa.
A copy of return ticket.
Bank statement dating back at least 3 months to show that you can fend for yourself during your time in Denmark.
Applicants should ensure they have a copy of the correctly filled and signed visa application form.
